There has to be at least one other player in the same session to find the Phantom Car. Phantom Car will arrive after at least 16 minutes have passed since the free roam session started. With this mod, the cars are very fragile now and you can't just drive and crashed at 100Km/h then kick the throttle and rolling again. So once again for those who seek realism, you should have this mod installed.
